PointClickCare Integration with WelcomeHome
WelcomeHome has integrated with PointClickCare to allow for one-touch transfer of information about prospective residents to PointClickCare and continuous updates of resident’s Admit, Discharge, and Transfer data from PointClickCare into WelcomeHome’s Occupancy Board and prospect files.
Data Flows
WelcomeHome will initially populate PointClickCare with the following data
- First Name*
- Middle Name
- Last Name*
- Gender*
- Date of Birth*
Note: * = Required
Once the patient record is accepted in PointClickCare, additional information is shared from WelcomeHome concerning influencers, including:
- Address
- Home/Mobile/Work Phone Numbers
- Email Address(es)
- Gender
- Date of Birth
- Relationship
In addition, we will send any available information on:
- Room / Unit
- Move-In Date
From PointClickCare, WelcomeHome receives:
- Admission Date
- Leave Date (recorded in WelcomeHome as “On leave”)
- Return from Leave Date
- Transfer Date
- Transfer To/From
- Room Reserve Date
- Room Reserve Location
- Stop Billing Date
- Move-Out Date
- Move-Out Reason
- Move-Out Reason Details
Key Touchpoints
Syncing/Sending a Prospective Resident to PointClickCare
To send a prospective resident from WelcomeHome to PointClickCare, a WelcomeHome user merely clicks on the “Sync to PointClickCare” button on the Prospect Summary.
All prospective residents are sent to the External Pending tab in PointClickCare. You or your team must accept the prospect in PointClickCare for additional information to be shared between the systems.
Once complete, the Prospect Summary within WelcomeHome will indicate that the sync is active.
Receiving Events from PointClickCare into WelcomeHome
All Admit, Discharge, and Transfer events created in PointClickCare are automatically updated in WelcomeHome.
These events are recorded in multiple places – in the Resident History, in the Housing Contract, and on the Occupancy Board.
