Eldermark is an Electronic Medical Records (EMR) billing system that your operator can use to share resident information with WelcomeHome. The information shared will be related to their resident information and status, which can be grouped in three categories of Admissions, Discharges and Transfers (ADTs). Once integrated, information will flow bi-directionally between Eldermark and WelcomeHome.
Syncing & Making Changes with Eldermark
As discussed in the EMR Overview article, you can sync a prospect to Eldermark only once. WelcomeHome requires the first name, last name and unit to sync, but we recommend adding in as much information as you can and waiting until the move-in details are confirmed before syncing. This is important to keep in mind when you have multiple people on one profile. If you sync an individual, then go in and add a partner or spouse, you will not be able to sync again to include that person. Again, it is best practice to wait until you have all the information confirmed for all parties before you sync.
Once synced, you'll want to make all changes in Eldermark and allow that information to push over to WelcomeHome. Each night ~9 PM EST, the integration will fetch the Resident Unit Histories for every resident in WelcomeHome that’s been synced with Eldermark. If the histories contain changes since the previous night’s sync, the integration will update the resident's housing contracts accordingly. The fields that can be set are:
- Unit #
- Privacy Level
- Privacy Level Variant - from Eldermark value Is_Second_Occupant
- Move In Date
- Move Out Date
- Count Move In - from Eldermark value Move_in_is_Transfer
- Count Move Out - from Eldermark value Move_out_is_Transfer
- Notice Given Date
- Care Type
Finding a resident
When a resident is initially synced, a resident profile will populate in PCC where all ongoing changes need to be made. Residents will flow into Eldermark’s Resident Chart section and are also discoverable via the Marketing Census Rollup.
To locate the Marketing Census Rollup, head to the Reports section, then Census Reports, where you will find the Marketing Census Rollup report.
After selecting the report, load and filter communities as needed.
To drill down on residents, click on the number in the Move Ins row to see individual residents.
